  1. Myocardial disease
    • ischemia
    • infiltrative (e.g. amyloidosis)
    • dilated cardiomyopathy
    • myocarditis
  2. Pericardial effusion
  3. Thick chest wall / barrel chest: COPD, obesity
  4. Generalized edema
  5. Hypothyroidism/myxedema
  6. Inappropriate voltage standardization
